Referral
When considering a referral into SAR, you may wish to first raise a safeguarding concern, to ensure you have addressed the incident with the local authority.

Please complete this form with the details of the case you wish to refer to the Merseyside Safeguarding Adults Review Group (MSARG) for consideration of a Safeguarding Adult Review (SAR). Please summarise why you feel it meets the 3 criteria for a SAR.

Criteria for conducting Safeguarding Adults Reviews. Section 44 of the Care Act 2014 requires that a Safeguarding Adults Board (SAB) must arrange for there to be a Safeguarding Adult Review (SAR)
This is when an adult in its area with needs for care and support (whether or not the local authority has been meeting any of those needs) if,
there is reasonable cause for concern about how the Safeguarding
Adults Board, members of it or other persons with relevant functions
worked together to safeguard the adult, and
- the adult has died, and
- the SAB knows or suspects that the death resulted from abuse or neglect (whether or not it knew about or suspected the abuse or neglect before the adult died)
or,
- the adult is still alive and the Safeguarding Adults Board knows or suspects that the adult has experienced serious abuse or neglect.
An SAB may arrange for there to be a review of any other cause involving a adult in its area with needs for care and support (whether or not the local authority has been meeting any of those needs.
Each member of the SAB must co-operate in and contribute to the carrying out of a review under this section with a view to –
a. identifying the lessons to be learnt form the adult’s case, and
b. applying those lessons to future cases.’
The MSARG will consider referrals at their monthly meeting, using the criteria outlined above. A recommendation is then made by the group for consideration by the Independent Chair of Wirral Safeguarding Adults Partnership Board. The Independent Chair’s decision is final.
