MSAR1 Deborah

Deborah (a pseudonym) died in December 2018. She was 54. She had been diagnosed with early onset dementia in 2014 and received specialist support on the Wirral where she resided with her husband* and teenage children. Safeguarding concerns arose periodically over the care her husband provided to her and considerable conflict arose between Deborah’s birth family (her mother, sisters and her brother) and her husband. Deborah’s birth family expressed repeated concerns that her care and treatment was being neglected by her husband. Her husband arranged for Deborah to be cared for by his family in the country of his birth (This country is outside the UK and will be referred to in this report as ‘Country A’) for substantial periods of time and in February 2017 Deborah went to stay with her mother-in-law in Country A and remained there until her death. Deborah’s birth family applied to the Court of Protection for Deborah to be repatriated to the UK but her deteriorating health was a key factor which prevented this from happening.

MSAR5  Tina

Tina, aged 61yrs, died in Hospital on 20th November 2017. She had been
taken by ambulance on the evening of 19th November to just a day and a
half after moving to an emergency care placement in Knowsley. Tina had
been found choking and unresponsive by a member of staff, who had left
Tina alone briefly whilst she was eating. Tina was placed on life support
when she arrived at Hospital but died the following day. The cause of her
death was recorded at the Coroner’s Inquest on 16th December 2021 as
‘Hypoxic’ brain injury; cardiac arrest; and choking on food.

MSAR 7  Adult 7

A referral to Adult Safeguarding Services, Wirral Care and Health Trust, was received in relation to a 94yr old gentleman (Adult 7) by North West Ambulance Service when they attended the Care Home where Adult 7 resided, in January 2019.  The referral outlined concerns at the lack of staff available to provide information in relation to Adult 7 who had fallen and sustained a laceration to his head on the 14 January 2019.  Adult 7 was taken to Arrowe Park Hospital where he subsequently died on the 17 January 2019.

MSAR8 Adult 8

Adult 8 was admitted from her own home Wirral University Teaching Hospital in August 2018.  Following a period of treatment at the acute hospital, Adult 8 was transferred to a care home in a T2A bed in September 2018.   During this time Adult 8 had a total of ten recorded unwitnessed falls and subsequently died from her injuries in October 2018.

MSAR24 Terry

Terry was a 63 year old white British man who died  by suicide at his home in March 2022.   The Ambulance Service had found Terry slumped on his sofa surrounded by vodka bottles; an undated note was nearby which described how unhappy he was with his life.  His sister had initially found him and called the Ambulance.   She was able to access his house because his front door was unlocked but she stated he often left it that way.   There were no signs of disturbance or third party involvement.    Terry had a background history of excessive alcohol use, agoraphobia and self-neglect.
